Reporting & monitoring

A short survey to keep your delivery data up to date through the year, so you’re not starting from scratch when the Annual Grant Report opens

Your once-a-year return: what the grant achieved, participant numbers, finance and outcomes. Opens 15 Aug, closes 15 Sept 2026 — this link is for 25–26 activity only.

A short, separate staff diversity return in line with Arts Council England requirements. Kept apart from your main report and seen only by Octagon’s data lead.

Delivering CPD or training? Ask attendees to fill this in straight after each session — it captures quality and confidence at the session level, which the Annual Report doesn’t. For 26.27 this is a mandatory template for all OMH funded CPD and Music Education Workforce capacity building activity.


Guidance & Templates

Fill these in as you go through the year – it’s a lot easier than rebuilding the numbers from scratch in September.

A plain-English guide to the Annual Survey defining things like “school engagement,” and “Classroom Instrumental Learning”.

Use this to track your partners, grant income and expenditure through the year and at the end of the year. Filling it in as you go it is the easiest way to answer the Finance section of your Annual Grant Report accurately.

If your grant funds delivery across a number of schools, use this template rather than entering totals school-by-school in the Annual Grant Report survey. This is a pre-populated version with every school in Dorset or Somerset.

For providers whose numbers in any single answer might reach 200 or more. Covers both Groups & Ensembles and Lessons, matched question-by-question to the live survey so you can complete it alongside your return and upload it in one go.
